Course Extension Request Form

Please read the following information in its entirety before filling out the form.

Independent Study Idaho allows for one four-month extension from the original end date of a course to be purchased at a cost of $110 before the course expires. Use this secure online form to request a time extension for the Independent Study in Idaho course(s) you are taking.

INSTRUCTIONS

1. Click Apply Now to begin.
2. Login to Keep Learning if you're not already. See login help if needed.
3. Your Keep Learning profile will be displayed, click Continue Checkout to continue.
4. Click Start to open the form.
5. Fillout the required fields, and press Submit Application.
